I, the undersigned parent or legal guardian of the participant mentioned earlier, acknowledge and understand that participation in cheerleading activities, including stunting and jumps, involves inherent risks, including but not limited to physical injury. I agree to release, waive, and hold harmless BLPA, its staff, coaches, volunteers, and affiliates from any liability, claims, or causes of action arising from or related to participation in Stallion Sparklers. I acknowledge that Banning Lewis Academy Schools and employees are NOT responsible for any injuries and/or lost or stolen items while attending the Stallion Sparklers events. I certify that my child is in good health and has no conditions that would prevent safe participation. I authorize emergency medical treatment if necessary and understand that I am responsible for any resulting medical expenses. I certify that if my child has special needs that I will be present at the clinic and at the gameday performance. Photo Release: I also give permission for my child’s photo or video to be used in promotional materials, social media, or publications related to the BLPA cheer program.
