Waiver and Release of Liability I, the undersigned parent or legal guardian of the above-named participant, hereby give my full consent and approval for my child to participate in the Clay High School Youth Summer Camps. I understand that participation in athletic and recreational activities involves inherent risks, including, but not limited to, physical injury, illness, or property damage. I acknowledge that Clay High School, its coaches, staff, volunteers, and representatives will take reasonable precautions to ensure the safety of all participants, but cannot eliminate all risks associated with participation. In consideration for allowing my child to participate, I hereby agree to the following: Assumption of Risk: I fully understand and assume all risks associated with my child's participation in the camp, including but not limited to injury or illness. Release of Liability: I, on behalf of myself and my child, hereby release, waive, discharge, and hold harmless Clay High School, its athletic department, coaches, staff, volunteers, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness (including COVID-19 exposure) that may be sustained by my child while participating in camp activities. Medical Treatment Authorization: In the event of an injury or emergency, I authorize the camp staff to secure medical treatment for my child as deemed necessary. I understand that I am responsible for any and all medical expenses incurred. By signing below, I acknowledge that I have read and fully understand this waiver and release, and that I am voluntarily allowing my child to participate in the Clay High School Youth Summer Camps.
